Cleveland Health Care Center is the highest rated, most established health care facility in the Cleveland, Texas area exclusively serving the geriatric population of the area for more than fifty years. Cleveland Health Care Center, located in Liberty County and just 45 miles north of Houston, is a 142 unit housing facility for elderly individuals. We are centrally located by retail shopping, houses of worship, and medical services. We are located less than a half mile from Cleveland Regional Medical Center, with 5 pharmacies located within 1 mile of the facility.  We also have six churches within four miles, which include Christian Life Center, Central Baptist Church, Repair the Breach Ministries, and Galilee Baptist Church.
