Some of our services offered at Cleveland Health Care Center include physical, occupational, and speech therapy. We also offer advanced wound care, IV therapy, respiratory therapy, and skilled nursing services, 24-hours a day. Our therapists and staff work with residents to create individualized plans of care, assistance with activities of daily living, podiatry, psychological services, and specialized memory care services. Our dietary service provides nutritious meals and snacks throughout the day. Housekeeping and laundry services are provided daily. Also available are diabetes management, pain management, respite care services, and a variety of other services to meet the clinical, emotional, and social needs of our residents.

Cleveland Health Care Center offers short-term or long-term options to help your loved one recovery as quickly as possible so they can return home and to their day-to-day life. We offer inpatient and outpatient therapy and rehabilitation services to all families.

Following surgery, or injury or illness, our staff provides skilled nursing services, and therapy and rehabilitation your loved one needs. There are a number of reasons you may need our care including stroke, broken bone, or surgery—our facility Cleveland Health Care Center, provides individuals with the 24 hour support they need to allow for a faster and improved long term recovery.

We understand that the aging process or illness can strip independence away from our residents and we strive to help our residents live as independently as possible while maintaining their security and safety. Wound Care

Cleveland Health Care Center offers a comprehensive range of treatments to aid in the healing of chronic wounds caused by diabetic ulcers, pressure ulcers, radiation injuries, venous insufficiency and other conditions. We provide full-service care to treat patients with emergency and chronic wounds resulting from various health conditions.

Our occupational therapy programs are designed to make the activities of daily living easier and more comfortable for each patient. Our physical therapy programs are tailored to each patient to regain strength, improve mobility, manage pain, or adapt to a disability. A physical therapist will work with you one-on-one to boost your motivation and help you regain confidence in day-to-day actions.
